Breast Augmentation Houston

Breast augmentation, as part of a mommy makeover, is a very common procedure performed by Dr. Paul Gill. Dr. Gill finds women loose volume in their breasts due to affects of pregnancy, aging, or weight loss. A breast augmentation alone or in combination with a lift can provide substantial improvement in breast size and shape. Women of the Houston, Woodlands, and Conroe areas frequently present to Dr. Gill for evaluation for breast augmentation with the mommy makeover procedure.

As part of Dr. Gill’s evaluation, he will utilize a series of measurements known as biodimensional planning to assess the chestwall and breast characteristics. This will help to identify an optimal and safe implant size range for patient sizing. Dr. Gill combines this with a careful history to understand and assess a patient’s goals and expectations. Based on these measurements and careful history, implant sizing is performed in the office to allow patients to gain a realistic idea of what implant size will meet their goals.

Dr. Gill will then discuss the various choices associated with breast implants and guide you to the right decision for you:

  1. Saline versus Silicone Implants: Saline implants have been FDA approved for more than 20 years and are found to be very safe. Many patients find comfort in knowing that if the saline implant ruptures, it is immediately identifiable. The downside to saline is that it does not look nor feel as natural as silicone. In addition, it is more prone to rupture and wrinkling/rippling.For this reason many patients choose silicone, because of the more natural feel and more natural look, with less likelihood of rupture or wrinkling. The FDA recommends evaluation with breast imaging every two years to assess the integrity of the silicone implant.
  2. Above or Below Muscle Placement: Dr. Gill has a strong preference for under the muscle because it provides for better implant protection against capsular contracture and infection. It also provides for a more natural appearing upper pole of the breast. Inpatients who are bodybuilders or professional athletes, Dr. Gill will recommend above the muscle to minimize malposition of the implant. Also, if a patient needs and a small breast lift, but does not want the extra scar, an above muscle implant placement may be an option. In general, Dr. Gill will place a smooth implant for his under the muscle breast augmentations and a textured implant for his above the muscle breast augmentation procedures.
  3. Incision choice: There are multiple incision options. The common incision choices utilized by Dr. Gill are:
    • Periareolar approach: this is a common approach for women who desire to hide the incision within the areola. It provides nice camouflaging of the incision, however the patient is more prone to sensation change and milk duct damage, which can also increase risk of infection.
    • Inframammary approach: this is the most common approach in Dr. Gill’s practice. It basically involves designing the implant incision within the crease of the breast. This approach carries much less risk of sensation change, milk duct damage, reducing the risk of capsular contracture and infection.
    • Breast List approach: If a breast lift is necessary, Dr. Gill will try to place the implants within this incision to minimize scarring. With a careful in office evaluation, Dr. Gill can determine if you will benefit from a primary breast augmentation or breast lift with augmentation.

Recovery:

Once you have completed the sizing process and Dr. Gill has reviewed his recommendations for your procedure, he will discuss the recovery process. This will depend on the type of breast augmentation performed, and any additional procedures performed, such as a mommy makeover.

The preoperative process always begins by performing preoperative lab work and if necessary a screening mammogram. We will then arrange a surgery day and preoperative visit. The preop visit is very helpful in easing any presurgery anxiety and it allows the patient and family to return to the office where Dr. Gill can answer further questions and solidify an operative plan. It is also a time to review the preoperative testing and give prescriptions prior to surgery.

The primary breast augmentation procedure takes anywhere from 30 to 45 minutes in surgery and 1-2 hours in the recovery room to ensure you are comfortable to return home. Dr. Gill performs all these procedures in a hospital or a surgery center adjacent to the hospital, for optimal patient safety. We find patients are sore for 3 to 5 days. In general, by the one-week visit, patients are usually off of all pain medication, and able to return to most of their daily activities. Dr. Gill does limit heavy pectoralis major muscle contraction for 1 month, which can cause implant displacement. Dr. Gill will see you for a 1 week, 3 week, and 6 week postoperative visit. This allows him to closely monitor your progress and to teach you the massage exercises while carefully following the incision for optimal healing. Generally, by six weeks, the implants have settled into position and the majority of swelling is down. Dr. Gill will then recommend that you are seen every three months for one year.
